Quality Health Solutions Selects Valence Health to Provide Population Health Management and Clinica l Integration Solutions

Valence Health, a leading supplier of population management, clinical integration and value-based care solutions, today announced it was selected by Quality Health Solutions, Inc. (QHS) to provide population health management and clinical integration solutions. QHS is an organization formed to support seven health care systems and the Medical College of Wisconsin who are working together as a virtual health care network to launch an accountable care strategy and other health care initiatives.

QHS represents 28 hospitals in Wisconsin, Michigan’s Upper Peninsula and northern Illinois, 4,000 affiliated physicians and combined net revenue of nearly $6 billion. Through a common technology platform and other initiatives, QHS seeks to drive the necessary collaboration to standardize quality and drive down health care costs across all care settings within its member organizations. With help from Valence Health, QHS member providers – who would otherwise operate independently – will become fully clinically integrated, adhere to common quality standards and be linked by a common clinical information platform to control costs and ensure high quality care. Once integrated, providers across the service area will have access to a patient’s complete history for services he or she received at any provider within the network – an important knowledge base for providing best possible care.

Under the agreement, Valence will provide QHS both the Valence VISION and vQuest solutions. As such, data from more than 50 practice management and hospital information systems and dozens of EMRs will be combined with lab, prescription and other data sources, to provide a comprehensive clinical viewof patients supported by QHS members. Once operational, the system will allow care providers and coordinators to identify customized patient populations based on combinations of clinical information–such as conditions, gaps in care per evidence-based guidelines, lab results, vitals and more. At the same time, Valence collects, aggregates, and analyzes financial information, giving providers a broad and detailed view to the cost of defined populations which enhances the ability of providers to be accountable for the care they provide.
